Who can request proxy access? |
Limited access via Nuance- Only Parent or Legal Guardian -If your child is age 0-17 years, you will be granted full access to your child's record. Once child reaches age 14 years, he/she will be able to access his/her own mySRH record pending the approval of the parent/guardian. Once child reaches age 18 years, parent/guardian will no longer have access to child's mySRH record. Parents with an adult child with diminished capacity who needs to retain proxy access should be instructed to do the following: In MyChart Menu, select “Ask A Question” Select Legal Guardianship documents Type a message regarding the patient’s incapacity and need to retain proxy access. Also attach any legal guardianship documentation This will be routed to the HIM Epic Inbasket for review and processing. If any additional information is needed, HIM will contact the proxy. |
